Responded to MVC that was upgraded to MVC with Rescue at location denoted.  Arrived to find Ford F250 work truck in ditch on left side.  AC 1 (Gladwell) established command as  crews of T1 and R16 began vehicular stabilization using chocks and Rescue 42 struts.  Entry was made into patient compartment to initiate EMS. Crews found a single male victim trapped under steering wheel.  Patient was initially alert and oriented, speaking to crews.  Patient's condition then began decreasing rapidly.  Crews utilized Hurst tools to make cuts to the right side A/B posts and began cutting the roof with a sawzall to remove  the roof panel in order to remove the patient from steering wheel. Upon removal, patient was moved to A162 for medical care and rapid transport to PRMC.  Patient's condition continued to deteriorate.  Patient went into arrest en route to PRMC.  Crews performed ALS interventions in an effort to revive patient. Paramedics on board provided care which brought the patient from full arrest to having a viable blood pressure and pulse at the time of transfer of care at the hospital. At last report, obtained notice that the patient required two surgeries and has survived and in ICU at this point.
